You do realize that sandbox games still have a plot. It is just not a straight-forward linear plot.

That is pretty much how all of my games are. I give them a large setting (which is just the land they are in) and a basic plot (bad guy/monsters/natural disaster/etc). Which is basically what you are calling the setting.
