I have seen some amazing stuff on here. The all time winner was a guy who wanted to kill himself so figured if he mixed multiple methods he would succeed.

He started by swallowing a bunch of sleeping pills, followed by covering himself in gas and setting himself on fire, and then the grand finale, he jumped off a 200ft cliff.

Small problem.... The cliff landed him in the ocean, which put out the fire; where he swallowed so much water that he puked up all the sleeping pills, leaving him with two broken legs and an interesting story for the life guards and EMT's as they pulled him out of the water....
