I think i would make it an MMORPG with a fairly large gameworld. Maybe make a gameworld divided into continents and make it diverse in landscapes ranging from tropical beaches, jungels, urban areas, rural areas and snow covered mountain ranges.
The type of monsters would come in a wide variety, but what kind of monsters it would be would depend on wether it was set in a dark medieval world or some kind of dark sci-fi future world. I am equally drawn to both genres. :)
I wish i could start to explain how i would do the skill system and how i would incorporate the quests into the storyline, but im running out of space to answer here :)

I hate that there is one armor set that you have to have at a certain level.
